Disability claims | A new standardized form
The Canadian life and health insurance industry has created a new standardized form to simplify the process and reduce the administrative burden on doctors. This form is used to collect the initial medical information that’s required for processing short- and long-term disability claims.
Always using the same form will help doctors become familiar with it. This will make it easier and faster for them to fill it out, which will speed up claims processing.
The new form is available now.
This initiative could provide faster access to benefits for people who need to take time off work for medical reasons.
Forms required for a disability claim
Two forms* must be completed:
- Standardized form: Initial Disability Insurance Medical Statement – 020255A
Starting February 16, 2026, the old form will no longer be accepted. - Disability Claim – Disabled Person’s Statement – 17001E
* Additional documents or information may be required during the claims analysis.
